Description:A registered nurse who coordinates the new patient interview process. Responsible for obtaining a telephonic cancer treatment history from patients before they arrive at the center. Provides clinical assessment services to referral sources, families and patients to determine the appropriate level of care needed for potential services within Cancer Treatment Centers of America.Uses evidence based clinical knowledge to ascertain appropriateness of patient for evaluation and treatment at Cancer Treatment Centers of America. Assesses for special travel and medical needs while conducting interviews and make sure schedulers are kept apprised of these needs. Possesses excellent telephonic customer service skills. Work very closely with other team members in an effort to provide the most integrative model of care to our patients. Responsible for reviewing new chart created by Intake Coordinator, abstracting chart and ensure that all necessary items are in place for appointment with intake physician. Responsible for the initial in person health assessment, medication review with patient and obtaining vital signs and entering all information into electronic health record.

  • Current Arizona RN license from an accredited college
  • BLS/CPR certified
  • BSN in Nursing preferred or Case Management certification with Associates Degree in Nursing
  • 3 years clinical oncology experience
  • 1 year experience in Case Management and/or similar role
  • Strong organizational and interpersonal skills
  • Current ONS Chemotherapy/Biotherapy provider card or able to attain during orientation at CTCA
  • Must possess the ability to utilize a clinical reasoning process for assessing, planning, implementing and evaluating the plan of care while ensuring that coordination of services are done in a timely manner.
  • Demonstrates sound knowledge and actions in the care and decision making for designated patient populations and seeks guidance appropriately.
  • Possesses a working knowledge and familiarity with applicable software packages.
